MINOR RELEASE FORM

Minor License Agreement and Release(Required)

The undersigned Parent or Legal Guardian hereby acknowledges and grants to Camp Trident the right and license from date of the camp to use the child’s name on camera footage and or still photographs and voice to be used for its fundraising and camp promotions.

I (we) here by certifies to Producer that he/she is over the age of 18 years, has full power, right and authority to enter into this Agreement and understands all of its terms and provisions.

We do not have enough instructors to facilitate a camper who is not able to make excursions from the main location of the camp. This Camp provides multiple experiences that require daily travel from the main camps location. Not allowing your child to travel from camp would limit the campers experience.
